Sample Shipping to the Stanford-SLAC CryoEM Facilities

This form will generate two labels. One for sample shipment to our CryoEM facility and one for the return shipment of the sample box or dewar to the user home institution. Please make arrangements with your local contact if you would like your samples to be stored at our facility. All samples must accompany a return label.

  • Note that deliveries are not accepted by SLAC on the weekend or holidays. Plan your shipment accordingly.
  • Fill-out the form below to obtain the required shipping labels and return shipping form. These must be attached to your sample box/dewar. (If applicable, remove all old shipping labels from your sample box/dewar)
  • All sample dewars and boxes will be inspected for integrity upon arrival.
  • Sample will be sent back in the original shipping container after measurement.

Sample boxes and dewars are received at SLAC and are usually delivered to our facility by 11 AM. Cryo-EM staff will check sample boxes and dewars between 11 AM and 2 PM. An e-mail will be sent to the address above when the sample boxes and dewars are checked in.

After submitting this form a PDF file will be created that includes your shipping labels and return shipping forms. Save this file, print these forms, attach the shipping labels to each dewar container and insert the return shipping forms inside each container.  Our shipping address is also listed below for your convenience.
