That's correct, when there isn't a couple out at the top; similarly, if
there isn't a couple out at the bottom, the bottom #1 lady should stay put
for the chain and then do the B2 part with the couple opposite. In general,
as this is a symmetrical dance, it's probably best to think about the end
effects only in terms of "couple out" or "couple not out" and forget whether
the end in question is the top or the bottom.

What about the couple who is out at the bottom - would they do the ladies

The general MUC end effect rule of thumb (which works perfectly for this
dance, though you can find a few dances where it breaks down) is: "Do
everything you can do without dancing with ghosts. If you'd have to dance
with ghosts to do something, stand still instead."
