SLAC Employees:

1. If you are paying for Registration and lunches (and/or banquet) please fill out the form, proceed to the secure payment site where you will only pay for your lunches (and/or banquet). Then to have your registration paid by STAP or an internal transfer please fill out this Excel sheet and send it to Training MS 11.

2. If you are only paying for Registration and no meals please fill in this form so we know you are coming but do not proceed to the secure payment page simply fill out this Excel sheet and send it to Training MS 11.

3. If you are paying for Registration and any meals with a check or a personal credit card please continue with this form and proceed to the secure payment page.
