STAP Training Funds/SLAC Account   (SU/SLAC staff -reg fees only)  SLAC Staff: A SLAC Staff Training Request must be submitted for use of STAP and division funds. First, print a copy of this registration page and hit the submit button before using the link to the Staff Training Request page. Then disregard the instructions on the bottom of the SLAC Training Request form to complete and attach the SLAC Registration Payment form, and instead forward a copy of this registration page together with your approved SLAC Training Request form to S. Carlson (MS: 69).  SU campus staff: you must pay for this and be reimbursed through the campus STAP process

For all payments by check, please be sure to include name of registrant with the check so we can match registration forms and checks appropriately. Payments must be postmarked before Sept. 30 to be eligible for early registration.
